An exciting opportunity has arisen for two dynamic Consultant General Physicians with an interest in Geriatric Medicine to join a progressive and expanding Older Persons' Medicine service in Wales. These substantive posts are part of a major transformation programme designed to improve acute frailty care, integrated community pathways, and inpatient geriatric services.

The successful candidates will join an established Consultant team delivering high-quality care for older people across acute, frailty, rehabilitation, and community settings. Applicants with 1 year of NHS experience are encouraged to apply.

Main Duties of the Job

  • Provide Consultant-led Geriatric Medicine and General Internal Medicine services.
  • Deliver acute frailty care through a single point of access, an integrated hub, and a co-located frailty unit.
  • Lead rotational frailty intake and Same Day Emergency Care frailty services.
  • Provide senior clinical leadership to multidisciplinary teams managing older patients with frailty.
  • Support virtual wards, Hospital at Home, acute community teams, and admission avoidance pathways.
  • Undertake inpatient ward rounds and manage older people requiring longer admission or rehabilitation.
  • Participate in the general medical on-call rota.
  • Contribute to orthogeriatric, liaison, outpatient, and ambulatory geriatric services.
  • Supervise junior doctors, trainees, ACPs, and multidisciplinary staff.
  • Participate in undergraduate and postgraduate teaching.
  • Lead audit, quality improvement, governance, and service development initiatives.
  • Contribute to research and innovation within medicine for older people.

Person Specification

Qualifications

Essential

  • Full GMC Registration
  • Licence to Practise
  • MRCP or equivalent
  • On the GMC Specialist Register in Geriatric Medicine and General Internal Medicine, or eligible for CCT within 6 months of interview

Desirable

  • Higher degree (MD / MSc / PhD)
  • Teaching qualification / Medical Education qualification

Experience

Essential

  • Minimum 1 year NHS experience
  • Broad experience in Geriatric Medicine and General Internal Medicine
  • Experience managing acutely unwell older patients with frailty
  • Multidisciplinary working across acute and community settings
  • Evidence of teaching and training junior medical staff
  • Experience in completing clinical audit and quality improvement work
  • Service improvement experience with management and clinical colleagues

Desirable

  • Development and commissioning of frailty services
  • Research in Geriatric Medicine / General Internal Medicine
  • Experience in Hospital at Home / Virtual Wards / Interface Frailty Medicine

  • Strong leadership and clinical judgement
  • Excellent communication with patients, carers, relatives, and colleagues
  • Ability to lead multidisciplinary teams effectively
  • Clinical expertise in frailty, acute geriatrics, and general medicine
