We are seeking an experienced and forward-thinking Consultant Geriatrician to join a dynamic and innovative Geriatric Medicine team in the Greater London region. This is an excellent opportunity for a clinician passionate about improving the quality of care for older adults and frail patients.

The postholder will play a leading role in developing and delivering comprehensive geriatric care across acute, frailty, and community settings. You will work collaboratively with a multidisciplinary team to enhance patient flow, reduce unnecessary admissions, and support early discharge planning — all within a supportive and progressive clinical environment.

The department is embarking on an exciting transformation programme, introducing Frailty Same Day Emergency Care (SDEC) and an Enhanced Acute Frailty Unit (AFU), both designed to deliver proactive, compassionate, and personalised care to older adults.

Key Responsibilities

  • Provide high-quality consultant-led clinical care for older and frail patients across inpatient, outpatient, and community services.
  • Deliver Comprehensive Geriatric Assessments (CGA) and lead multidisciplinary discussions to ensure effective treatment and discharge planning.
  • Support and further develop the frailty and interface services across the Emergency Department and acute wards.
  • Contribute to the integration of hospital and community-based care through close collaboration with local health and social care partners.
  • Lead, mentor, and supervise junior medical staff to ensure excellent training and professional development.
  • Participate in clinical governance activities, including audit, research, and service improvement projects.
  • Take an active role in teaching undergraduate and postgraduate trainees, nursing staff, and allied health professionals.
  • Engage in departmental quality improvement initiatives and contribute to shaping frailty and elderly care services across the region.

Clinical Duties

  • Conduct regular ward rounds and multidisciplinary team meetings within Medicine for the Elderly.
  • Support the delivery and ongoing development of frailty models of care, including SDEC and short-stay units.
  • Respond to inpatient and outpatient referrals and provide specialist advice to colleagues and primary care partners.
  • Participate in the on-call rota as required.
  • Contribute to achieving national clinical targets such as four-hour Emergency Department performance and 18-week outpatient pathways.
  • Provide clinical leadership for service transformation projects in frailty and integrated care.

Education and Teaching

  • Supervise medical students and junior doctors, providing structured teaching and feedback.
  • Act as a Clinical and Educational Supervisor for trainees in Geriatric and General Medicine.
  • Support the delivery of teaching for undergraduate medical curricula in collaboration with local universities.
  • Participate in continuing professional development and revalidation requirements.

Research, Audit, and Governance

  • Participate actively in departmental audits, research projects, and service evaluations.
  • Support NIHR-funded and local research initiatives within Geriatric Medicine.
  • Promote evidence-based practice and contribute to clinical guideline development.
  • Participate in annual appraisal and continuing professional development to maintain GMC revalidation.

Management and Leadership

  • Work collaboratively with other consultants and service managers to plan and develop services for older people.
  • Contribute to the strategic direction of the department and wider organisation.
  • Participate in quality improvement, patient safety, and clinical governance committees as required.
  • Provide clinical leadership in areas of special interest such as frailty, dementia, falls, or community geriatrics.

Person Specification

Essential Qualifications

  • MBBS or equivalent medical qualification.
  • Full GMC registration with a licence to practise.
  • MRCP (UK) or equivalent postgraduate qualification.
  • Inclusion on the GMC Specialist Register for Geriatric Medicine, or within six months of obtaining CCT at the time of interview.

Experience

  • Broad-based training and experience in Geriatric and General Internal Medicine.
  • Proven ability to lead multidisciplinary teams and manage complex elderly patients.
  • Experience in teaching, training, and supervision of medical trainees.
  • Understanding of clinical governance and experience with clinical audit.

Desirable

  • Higher degree (MD/PhD or equivalent).
  • Experience in frailty service development or community geriatrics.
  • Demonstrated interest or experience in research, service innovation, or leadership roles.
