Geriatrics - Consultant
We are seeking a dynamic and motivated Consultant in Geriatric Medicine to join our multidisciplinary team, which provides high-quality care for older patients across the South East region. This fixed-term position, available until July 2026, includes maternity cover and is ideal for candidates available to commence at the earliest opportunity.
This is a key role within the acute frailty service, supporting the transition from age-related services to a needs-based model of care for frailty. The post-holder will contribute to the Frailty Assessment Unit and work closely with Emergency Department teams and community partners to ensure the delivery of timely and comprehensive care by the National Service Framework for Older People and NICE guidelines.
Key Responsibilities
- Deliver high standards of acute care to older people from the Emergency Department through to discharge, using frailty criteria rather than age-based referrals.
- Provide consultant input to the Frailty Assessment Unit and Emergency Department, supporting the shift towards an Acute Physician-led AMU model.
- Undertake the continuing clinical responsibility for patients under the Frailty Model, including related administrative duties.
- Collaborate with the Clinical Director and Care Group Management to support strategic, budgetary, and service development objectives.
- Participate in the general medical and rehabilitation on-call rota and contribute to the continuity of care.
- Engage in additional responsibilities, including clinical governance, committee participation, patient and family meetings, and management tasks.
- Work closely with the Integrated Discharge Team and community services to ensure seamless patient transitions and care.
Person Specification
Skills and Experience
Essential:
- NHS Experience Required
- Experience in Frailty and Emergency Department pathways
- Experience in service improvement and clinical audit
Qualifications and Training
Essential:
- Full GMC Registration
Desirable:
- Higher degree (MD, PhD, or equivalent) in Geriatric Medicine
Other Requirements
Essential:
- Ability and willingness to travel effectively between sites
- Commitment to continuous professional development and clinical leadership
