An exciting opportunity has arisen for a Consultant Physician in Geriatric Medicine with dual accreditation in General Internal Medicine and Geriatric Medicine to join an integrated healthcare service in Ireland.The successful candidate will provide consultant-led care for older adults across acute, ambulatory and community settings, delivering Comprehensive Geriatric Assessment (CGA), acute medical care, rehabilitation and frailty services. Working within multidisciplinary teams, the post holder will support integrated care pathways that promote admission avoidance, early supported discharge, and patient-centred care, while contributing to service development, education, research, and clinical governance.
Main Duties and Responsibilities
- Deliver a comprehensive Consultant-led Geriatric Medicine service across acute, ambulatory and community care settings.
- Provide specialist assessment, diagnosis and management of frail older adults with complex medical needs.
- Deliver Comprehensive Geriatric Assessment (CGA) to optimise patient outcomes and functional independence.
- Manage acute medical admissions involving older adults and participate in General Internal Medicine services.
- Participate in the General Medical on-call rota, including acute medical and stroke services where required.
- Support integrated care pathways that promote admission avoidance, early discharge and community-based management.
- Lead multidisciplinary team (MDT) meetings involving nursing, physiotherapy, occupational therapy, pharmacy, social work and other allied health professionals.
- Provide specialist geriatric consultation services to colleagues across medical and surgical specialities.
- Deliver outpatient clinics and follow-up services for older adults with complex geriatric conditions.
- Contribute to rehabilitation services and transitional care programmes for older patients.
- Participate in service planning, clinical governance, audit, quality improvement and patient safety initiatives.
- Supervise, mentor and provide educational support to Non-Consultant Hospital Doctors (NCHDs), medical students and multidisciplinary staff.
- Support the development of integrated services for older persons in line with national healthcare strategies.
- Maintain accurate clinical documentation and ensure compliance with Irish Medical Council and HSE professional standards.
Person Specification
Qualifications & Registration
Essential
- MBBS, MBChB or equivalent primary medical qualification.
- Specialist Registration with the Irish Medical Council (IMC) in General Internal Medicine and Geriatric Medicine, or eligibility to obtain Specialist Registration before appointment.
- Certificate of Satisfactory Completion of Specialist Training (CSCST), CCT or an equivalent recognised specialist qualification.
- Eligibility to practise as a Consultant Physician in Ireland.
Desirable
- Fellowship of the Royal College of Physicians or equivalent recognised postgraduate qualification.
- Additional postgraduate qualifications relevant to Geriatric Medicine, Stroke Medicine or Integrated Care.
- Higher academic qualification such as MD, MSc or PhD.
Experience
Essential
- Extensive Consultant-level or equivalent experience in Geriatric Medicine and General Internal Medicine.
- Experience managing frailty, multimorbidity and complex geriatric syndromes.
- Experience delivering Comprehensive Geriatric Assessment (CGA).
- Experience managing acute medical admissions involving older adults.
- Experience participating in General Internal Medicine and acute on-call services.
- Experience working within multidisciplinary teams across acute and community healthcare settings.
- Experience supervising and mentoring junior doctors.
- Experience participating in clinical governance, audit and quality improvement initiatives.
Desirable
- Experience developing integrated care services for older adults.
- Experience in stroke medicine or rehabilitation services.
- Research experience with publications in Geriatric Medicine or related specialities.
