An exciting opportunity has arisen for a Consultant Gastroenterologist to join a growing Adult Medicine service in the Channel Islands. This is a full-time, 10 PA-equivalent post created in response to increasing demand and the continued expansion of gastroenterology services. The successful appointee will join an established team and play a key role in shaping and developing local gastroenterology provision, while helping improve patient access, patient choice and the development of new on-island services.

The service has recently moved from a model in which gastroenterology was combined with General Internal Medicine to a dedicated gastroenterology structure, and this post represents the appointment of a second full-time gastroenterologist, with support from a Nurse Endoscopist and an IBD Specialist Nurse. This post carries no formal GIM on-call commitment and no formal on-call rota requirement.

Main Duties & Responsibilities

• Provide expert gastroenterology care across inpatient and outpatient settings.
• Deliver a comprehensive gastroenterology service, including assessment, diagnosis, treatment and ongoing management of both elective and emergency GI conditions.
• Undertake 2–3 general gastroenterology outpatient clinics per week, including hepatology referrals, excluding viral hepatitis services.
• Contribute to the delivery of endoscopy services, including upper GI endoscopy, flexible sigmoidoscopy, colonoscopy, PEG, PEJ and enteroscopy.
• Work collaboratively with colleagues and support staff to improve patient pathways, maintain safe waiting times and support service development.
• Support and advise on the management of patients under the care of other specialities where gastroenterology input is required.
• Act as clinical lead for the local bowel cancer screening programme on operational matters, including support and guidance for the Nurse Endoscopist where required
• Participate in GI MDT meetings, ward reviews, inpatient care and wider departmental clinical activities.
• Develop and maintain strong links with tertiary centres to support integrated patient care and appropriate onward referral where needed.
• Contribute to clinical governance, audit, quality improvement, guideline development and risk management.
• Support education and supervision of allied health professionals, nursing staff and medical students as required
• Maintain timely administration, accurate record keeping and effective use of designated electronic systems
• Participate in appraisal, revalidation, mandatory training and continuing medical education activities.

Service Overview

The successful appointee will join a well-established Adult Medicine service serving the local island population, with close working relationships across medicine, surgery and supporting specialities. Gastroenterology services are supported by:

• A Nurse Endoscopist
• An Inflammatory Bowel Disease Specialist Nurse
• Trained endoscopy nurses supporting therapeutic procedures, including PEG/PEJ work
• Strong support from radiology, histopathology, dietetics and speech & language therapy
• Established weekly GI MDT meetings covering GI cancer and non-malignant gastrointestinal disease
• Tertiary links with mainland centres for highly specialised work and complex referrals

Endoscopy & Gastroenterology Service Highlights

The gastroenterology department currently provides:

• Upper GI endoscopy, including therapeutic work such as stenting and banding
• Flexible sigmoidoscopy
• Colonoscopy
• PEG and PEJ insertion
• Enteroscopy

Some advanced procedures are currently referred off-island, including ERCP, EUS, oesophageal manometry, 24-hour pH studies and certain advanced therapeutic procedures. However, there is interest in further service development where expertise allows. One endoscopy list each week has anaesthetic support. The service also supports a local FIT-based bowel cancer screening programme and additional outpatient/day-unit activity for therapies such as iron infusions, biologic treatment and other non-endoscopic GI procedures.

Requirements

Essential

• Entry on the GMC Specialist Register for Gastroenterology
• CCT in Gastroenterology or expected entry within 6 months of interview, or CESR / European Community Rights
• MRCP or equivalent
• Eligible to reside and work in the UK
• Clinical training and experience equivalent to that required for gaining a UK CCT in Gastroenterology
• Ability to offer expert clinical opinion across a broad range of elective and emergency gastroenterology presentations
• Ability to take full and independent responsibility for the clinical care of patients
• Ability to integrate patient care appropriately with tertiary centres and develop effective professional links
• Ability to act as clinical lead for the bowel cancer screening programme on operational matters
• Evidence of commitment to clinical governance, service development, organisation of outpatient priorities and administrative responsibility
• Ability to teach clinical skills to nursing, technical staff and medical students
• Ability to apply research outcomes to clinical practice
• Strong communication skills, team-working ability, flexibility and a caring patient-centred approach

Desirable

• Entry on the GMC Specialist Register for General Internal Medicine
• Higher degree
• JAG / BCSP accreditation
• More than 1 year of experience at the consultant level
• Ability to develop a sub-speciality interest
• Ability to perform ERCP
• Management training or experience in managing a clinical service
• Experience with digital systems and service improvement
• Instructor status on a recognised course
• Publications or research activity
• Willingness to undertake additional responsibilities and contribute to long-term service development
