An exciting opportunity has arisen for an Advanced Clinical Practitioner (ACP) to join a dynamic healthcare team in Greater London. This role is integral to ensuring high-quality, patient-centred care while supporting clinical decision-making and leading service provision in the absence of Service Leads.
As an ACP, you will work autonomously within a multi-disciplinary team, demonstrating advanced clinical skills, strong leadership, and a commitment to service improvement. The role also includes mentorship, clinical supervision, and supporting the professional development of junior staff.
Key Responsibilities
- Serve as the clinical lead on duty, advising and supporting clinical decision-making while acting as the point of escalation as shift coordinator.
- Take charge of shifts and oversee service provision when Service Leads are absent.
- Conduct comprehensive health assessments, including history-taking, clinical examination, diagnosis, and treatment.
- Ensure safe management, referral, or discharge of patients based on assessment findings.
- Support and mentor junior staff, fostering clinical development and competency growth.
- Oversee patient flow within the department, ensuring smooth operations and a positive patient experience.
- Identify the need for diagnostic tests, interpret results, and take appropriate clinical action.
- Lead on clinical audits, ensuring services align with NICE guidelines and best practices.
- Promote infection prevention and control measures in line with policies.
- Work collaboratively across primary and secondary care to improve patient pathways and prevent unnecessary hospital admissions.
Person Specification
Essential Criteria:
- Registration with a relevant clinical body (NMC/HCPC).
- Master’s degree in Advanced Clinical Practice (ACP).
- Independent Non-Medical Prescribing Qualification (V300).
- Mentor/teaching qualification.
- Experience working autonomously in an advanced clinical role.
- Strong leadership skills with experience in service coordination.
- Ability to conduct clinical research and audits.
Desirable Criteria:
- Community healthcare experience within a multi-disciplinary team setting.
- Proven experience in education and staff training.
