An exciting opportunity has arisen for an Advanced Clinical Practitioner (ACP) to join a dynamic healthcare team in Greater London. This role is integral to ensuring high-quality, patient-centred care while supporting clinical decision-making and leading service provision in the absence of Service Leads.

As an ACP, you will work autonomously within a multi-disciplinary team, demonstrating advanced clinical skills, strong leadership, and a commitment to service improvement. The role also includes mentorship, clinical supervision, and supporting the professional development of junior staff.

 

Key Responsibilities

  • Serve as the clinical lead on duty, advising and supporting clinical decision-making while acting as the point of escalation as shift coordinator.
  • Take charge of shifts and oversee service provision when Service Leads are absent.
  • Conduct comprehensive health assessments, including history-taking, clinical examination, diagnosis, and treatment.
  • Ensure safe management, referral, or discharge of patients based on assessment findings.
  • Support and mentor junior staff, fostering clinical development and competency growth.
  • Oversee patient flow within the department, ensuring smooth operations and a positive patient experience.
  • Identify the need for diagnostic tests, interpret results, and take appropriate clinical action.
  • Lead on clinical audits, ensuring services align with NICE guidelines and best practices.
  • Promote infection prevention and control measures in line with policies.
  • Work collaboratively across primary and secondary care to improve patient pathways and prevent unnecessary hospital admissions.

Person Specification

Essential Criteria:

  • Registration with a relevant clinical body (NMC/HCPC).
  • Master’s degree in Advanced Clinical Practice (ACP).
  • Independent Non-Medical Prescribing Qualification (V300).
  • Mentor/teaching qualification.
  • Experience working autonomously in an advanced clinical role.
  • Strong leadership skills with experience in service coordination.
  • Ability to conduct clinical research and audits.

Desirable Criteria:

  • Community healthcare experience within a multi-disciplinary team setting.
  • Proven experience in education and staff training.
