The NHS offers a wide range of career options, but the structure is not always easy to navigate. Understanding how progression works can help you make clearer, better-informed decisions about what comes next.

A simple view of how careers progress.
At a high level, many doctors move through:
Foundation → Core Training → Specialty Training → Consultant
But this is not the only route.

Foundation Training
Broad clinical exposure and early development.
Core Training / Early Career
A stage of deeper direction-setting and skills development.
Specialty Training / Registrar Level
Greater specialisation, responsibility and preparation for senior roles.
Consultant Level
Senior clinical leadership, service contribution and supervision responsibilities.
There is more than one way to progress.
In addition to the traditional training pathway, doctors may also progress through:
- SAS and Specialty Doctor roles
